This card is a time schedule for typhoid and para-typhoid inoculations and when properly filled in and signed is an official record of their completion. The Typhoid and para-typhoid inoculations combined are given in three doses at intervals of about seven days. Dates on cards indicate when the man should report for treatment. Failure to so report renders him liable to court martial and necessitates the repetition of the inoculation. If no notice to report is received on the date specified he should notify his commanding officer and when reporting for treatment bring the card with him and see that the date is properly initialed, and signed by a medical officer upon completion. KEEP THIS CARD!!!
