Subject: Allotment and Allowance # A2087318
I hereby certify that I, George Levi McCormac, on November 1st. 1918, while a member of the 658th. Aero Squadron, grade of Corporal, Army Serial No. 35740, then stationed at 2nd. Reserve Park, Voltaire, Paris, France, made allotment of additional Family Allowance to my Father, William Hirst McCormac, and my Mother, Josie Lee McCormac. Since which said date, up to and uncluding April 1919, each month $15.00 has been deducted from my pay.
I was discharged from the service May 20th. 1919, at Camp Dodge, Des Moines, Iowa, $10.00 being deducted from my final pay to apply on my allotment for the month of May.
On May 27th. 1919 my Father, William H. McCormac received draft from Treasury Department for the sum of $75.00 "as payment of allotment and family allowance # A2087318 for months of November to April inclusive," being the first and only sums of money he has received. To date my Mother, Josie Lee McCormac has received no sum or sums of money what so ever from my allotment or from the Government as Family Allowance.
Request I be notified if my claim for Family Allowance was approved, and that any sums of money due my said Father and Mother, either from my allotment of from the Government as Family Allowance be forwarded. Signed: George L. McCormac, Hunstville, Missouri.
